Bronze:
$2,500 for a two day exhibit space, 2 x 5 table top type. In this is included 1 pass for the session and invitation to all breakfasts and lunches. Extra passes may be purchased for $1,000 a pass. Limited table top exhibit spaces available

$5,000 for a 10 x 10 exhibit space. In this is included 2 passes for the session and invitation to all breakfasts and lunches. Extra passes may be purchased for $1,000 a pass.

Gold:
$10,000 will allow for one 10 x 20 or two 10 x 10 exhibit spaces at a premium location and 4 session passes or 2 waived attendance fees for sponsored physicians as well as invitations to all breakfasts and lunches. You will also receive recognition of corporate support on all promotional materials (print ads, e-blasts, official program) and sponsorship of one break.

Platinum:
$25,000 will allow for one 10 x 20 or two 10 x 10 exhibit spaces at a premium location and 6 session passes or 3 waived attendance fees for sponsored physicians as well as invitations to all breakfasts and lunches. You will also receive recognition of corporate support on all promotional materials (print ads, e-blasts, official program) and sponsorship of one break. Additionally, you will receive sponsorships of the Hospitality Suite, one break, and one lunch.

Diamond:
$35,000 will allow for one 10 x 20 or two 10 x 10 exhibit spaces at a premium location and 10 session passes or 5 waived attendance fees for sponsored physicians as well as invitations to all breakfasts and lunches. You will also receive recognition of corporate support on all promotional materials (print ads, e-blasts, official program) and sponsorship of one break. Additionally, you will receive sponsorships of the Hospitality Suite, one break, one lunch or faculty dinner, and airport pickup for faculty.

Refund Policy
Your registration will be refunded 50% up until 11-3-2011. After this date, no refunds will be made.
